Our organization offers Medicaid-covered Community Health Worker (CHW) services to support individuals in managing their health and navigating the healthcare system. Services include health coaching and education to manage all chronic health conditions, such as diabetes, high blood pressure, asthma, obesity, anxiety, substance use and more. CHWs provide personalized support to help individuals understand their conditions, follow care plans, and make healthy lifestyle changes. We also assist with connecting to healthcare providers, scheduling appointments, using telehealth, and accessing local resources. Additionally, health education groups are provided for all chronic health conditions. All services are based on medical need and must be authorized by a licensed healthcare provider.

Yes! Thriving Together's payor source is Medicaid Insurance. Currently, no other insurance(s) or payor methods are accepted.

The Thriving Together office is open from 8:00 AM - 4:00 PM. Weekends when group activities are run, the office is open 12:00PM - 2:00 PM. However, Community Health Worker Service hours differ among CHW's. Please contact us to see how we can meet your needs!
Yes. We have Community Health Worker's as well as a Clinical Social Worker who are bi-lingual. Additionally, interpreter services via telephone are available to bridge the gap among all languages.
